Long-Term Disability (LTD)
If you choose to enroll in LTD coverage, you and your spouse may be required to complete an Evidence of Insurability (EOI) questionnaire if you elect coverage outside of your initial new hire enrollment.

About pre-existing conditions
In general:
- No benefit will be paid if the claim occurs during the “pre-existing condition trial period” and is the result of an illness or accident connected to a “pre-existing condition.”
- Any illness or injury for which you have received treatment including: being hospitalized for, had surgery for, received therapy for, consulted a medical practitioner for or taken medication for during the “pre-existing condition occurrence period” immediately prior to your coverage effective date.
