Basic Life & AD&D

To provide financial protection for you and those who depend on you, VillageMD provides Basic Life and AD&D coverage at no cost to you.

As a benefits-eligible team member, you are automatically enrolled in basic coverage. The benefit amounts vary based on your position (staff member or provider):

Reductions of your elected benefit amount may occur (and/or spouse coverage may terminate) at a certain age. See Reliance Standard’s certificate of coverage and policy details.

Initial Enrollment
To enroll or increase coverage after your initial enrollment, Evidence of Insurability (EOI) will be required to elect more than $10,000 of new or increased employee coverage, or more than $5,000 of new or increased spouse coverage, up to guaranteed issuance amounts.

Terms to Know

  • Guaranteed Issuance (GI): The dollar amount of coverage for which you are guaranteed acceptance without providing EOI.
  • Evidence of Insurability (EOI): Reliance Standard will require proof of good health before approving your application for coverage above the guaranteed issuance level.
  • EOI PowerForm: This is the link you use to provide Evidence of Insurability when required.
